How to Use

Dosage

Your doctor will decide your dose based on why you need the medicine and your weight. A common dose for existing clots is 150 IU (1.5 mg) per kg once daily or 100 IU (1 mg) per kg twice daily. For prevention, doses are typically 2000 IU (20 mg) or 4000 IU (40 mg) once daily.

Method

This medicine is usually given as an injection under the skin (subcutaneously). It can also be given into a vein for certain heart attacks or after some surgeries, or into a tube during dialysis. Do not inject into a muscle.

Your doctor will determine the maximum daily dose for you based on your condition and weight, ensuring safe use.

Possible Side Effects

COMMON
  • Bleeding
  • Increased liver enzymes
  • Easier bruising (may be due to low platelet count)
  • Pink or red patches on the skin, often where injected
  • Rashes, hives
  • Itchy red skin
  • Bruising or pain at the injection site
  • Lower red blood cell count
  • Higher platelet count
  • Headache
RARE
  • Sudden severe headache, possibly a sign of bleeding in the brain
  • Stomach tenderness or swelling, possibly due to stomach bleeding
  • Large, red, irregular skin lesions, sometimes with blisters
  • Local skin irritation
  • Yellowing of skin or eyes, dark urine (signs of liver problems)
  • Severe allergic reactions (rash, difficulty swallowing or breathing, swelling of lips, face, throat, or tongue)
  • High potassium levels in blood (more likely with kidney problems or diabetes)
  • Increased levels of white blood cells called eosinophils
  • Hair loss
  • Bone weakening (osteoporosis) with long-term use
  • Tingling, numbness, and muscle weakness (especially in the lower body) after spinal procedures
  • Loss of bladder or bowel control
  • Hard lump or swelling at the injection site

Important Warnings

  • Do not switch this medicine with other similar blood thinners (low molecular weight heparins) without your doctor's guidance, as they are not exactly the same.
  • Stop using this medicine and get medical help right away if you have signs of a severe allergic reaction (like trouble breathing, swelling of lips, face, throat, or tongue) or a widespread red, peeling rash with bumps, blisters, and fever.
  • This medicine can cause bleeding, which can be serious. Contact your doctor immediately if you notice any bleeding that won't stop, or signs of heavy bleeding like unusual weakness, tiredness, paleness, dizziness, headache, or unexplained swelling.
  • Contact your doctor immediately if you notice signs of a new blood clot, such as cramping pain, redness, warmth, or swelling in one leg (deep vein thrombosis), or shortness of breath, chest pain, fainting, or coughing up blood (pulmonary embolism).

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe renal impairment. Consider potential interactions especially with other drugs that increase the risk of bleeding. (updated 19.9.2025)
