How to Use

Dosage

Your doctor will tell you your dose. For high blood pressure, the usual starting dose is 5-20 mg once daily, with a maintenance dose of 20 mg daily. For heart failure, the usual starting dose is 2.5 mg once daily, gradually increased by your doctor.

Method

Swallow the tablets with water. You can take them with or without food, preferably around the same time each day.

Important

The maximum daily dose is 40 mg, as prescribed by your doctor.

Possible Side Effects

COMMON
  • Dizziness, weakness, or feeling unwell
  • Blurred vision
  • Cough
  • Dizziness or fainting due to low blood pressure
  • Changes in heart rhythm, fast heart rate, or chest pain
  • Headache or depression
  • Changes in taste
  • Shortness of breath
  • Diarrhea or stomach pain
  • Fatigue
  • Skin rash
  • Allergic reactions with swelling of the face, lips, tongue, or throat, which may cause breathing or swallowing difficulties (angioedema)
  • High potassium or creatinine levels in blood (found in blood tests)
RARE
  • Redness or sudden drop in blood pressure
  • Rapid or irregular heartbeats (palpitations)
  • Heart attack or stroke (can happen due to very low blood pressure in some people)
  • Anemia (low red blood cells) or other changes in blood counts, bone marrow problems
  • Confusion, sleeplessness, drowsiness, nervousness, strange dreams
  • Tingling or numbness in the skin, vertigo (spinning sensation), ringing in the ears
  • Runny nose, sore throat, hoarseness, asthma-like chest tightness, lung inflammation, fluid in the lungs
  • Slow digestion, pancreatitis, nausea, vomiting, indigestion, constipation, loss of appetite, irritated stomach, dry mouth, stomach ulcer, swelling of the intestinal wall
  • Muscle cramps
  • Worsened kidney function or kidney failure, reduced urine output, high protein in urine
  • Increased sweating, itching or hives, hair loss, severe skin rashes (e.g., Stevens-Johnson syndrome), photosensitivity
  • General feeling of being unwell, fever, impotence
  • Low blood sugar or sodium, high urea in blood (found in blood tests)
  • Raynaud's phenomenon (hands and feet become very cold and white)
  • Enlarged lymph nodes or autoimmune diseases
  • Enlarged breasts in men (gynecomastia)
  • Liver or bile duct disorders (impaired function, hepatitis, jaundice, high liver enzymes)
  • Overproduction of antidiuretic hormone (causing fluid retention, weakness, fatigue, or confusion)
  • Syndrome including fever, blood vessel inflammation, muscle/joint pain, rash, or photosensitivity

Important Warnings

  • Do not take if you are allergic to enalapril maleate or other ACE inhibitors.
  • Stop immediately and contact a doctor if you experience swelling of the face, lips, tongue, or throat, or difficulty breathing/swallowing, or swelling in your hands/feet, or a red itchy rash.
  • Do not use if you are pregnant (especially after 3 months) or trying to get pregnant.
  • Do not use if you have diabetes or kidney problems and are taking aliskiren, a blood pressure medicine.
  • Do not use if you are taking a sacubitril/valsartan combination product for heart failure.

  • This medicine may cause dizziness or lightheadedness, especially when you start taking it. Lie down if this happens.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose already in mild renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
