This medicine helps lower high blood pressure by combining two ingredients: one that blocks a hormone that narrows blood vessels and another that helps your body get rid of extra water and salt.

How to Use

Dosage

The usual dose for most adults with high blood pressure is one Cozaar Comp 50 mg/12.5 mg tablet once a day.

Method

Swallow the tablets whole with a glass of water, with or without food.

Important

The maximum daily dose is two Cozaar Comp 50 mg/12.5 mg tablets or one Cozaar Comp Forte 100 mg/25 mg tablet.

Possible Side Effects

COMMON
  • Cough, upper respiratory infection, stuffy nose, sinus inflammation, sinus problems.
  • Diarrhea, stomach pain, nausea, indigestion.
  • Muscle pain or cramps, leg pain, back pain.
  • Sleeplessness, headache, dizziness.
  • Weakness, tiredness, chest pain.
  • High potassium levels (which can cause irregular heartbeat), low hemoglobin levels, changes in kidney function including kidney failure, low blood sugar levels.
RARE
  • Anemia, red or brownish spots on the skin (sometimes with joint pain, swelling of hands/feet, abdominal pain), bruising, low white blood cells, clotting problems, low platelet count.
  • Loss of appetite, high uric acid or gout, high blood sugar, electrolyte imbalance.
  • Anxiety, nervousness, panic disorder, confusion, depression, unusual dreams, sleep problems, drowsiness, memory problems, tingling or prickling sensations, limb pain, tremor, migraine, fainting.
  • Blurred vision, burning or stinging in the eyes, conjunctivitis (pink eye), decreased vision, yellow vision.
  • Ringing, buzzing, roaring, or clicking in the ears, spinning sensation (vertigo).
  • Low blood pressure (which can cause lightheadedness or weakness when standing up), chest pain (angina pectoris), irregular heartbeat, stroke (TIA, cerebrovascular disorder), heart attack, palpitations.
  • Blood vessel inflammation (often with rash or bruising), sore throat, shortness of breath, bronchitis, pneumonia, fluid in the lungs (causing breathing difficulties), nosebleed, runny nose, stuffy nose.
  • Constipation, severe constipation, gas, stomach upset, stomach cramps, vomiting, dry mouth, salivary gland inflammation, toothache, yellowing of eyes and skin (jaundice), pancreatitis, intestinal angioedema (swelling in the gut with stomach pain, nausea, vomiting, diarrhea), liver inflammation (hepatitis), abnormal liver function tests.
  • Hives, itching, skin inflammation, rash, skin redness, sun sensitivity, dry skin, flushing (warmth, redness), sweating, hair loss.
  • Pain in arms, shoulders, hips, knees, or other joints, joint swelling, stiffness, muscle weakness.
  • Frequent urination at night, abnormal kidney function, kidney inflammation, urinary tract infection, sugar in urine, decreased sexual desire, impotence.
  • Swelling of the face, local swelling (edema), fever.

Important Warnings

  • Do not take if you are allergic to losartan, hydrochlorothiazide, or any other ingredients in this medicine.
  • Do not take if you are allergic to other sulfonamide medicines (e.g., some antibiotics).
  • Do not take if you have severe liver problems.
  • Do not take if your potassium or sodium levels are too low, or calcium levels are too high and cannot be fixed by treatment.
  • Do not take if you have gout.

  • Do not take if you are more than 3 months pregnant.
  • Do not take if you have severe kidney problems or if your kidneys do not produce urine.
  • Do not take if you have diabetes or kidney problems and are taking another blood pressure medicine containing aliskiren.
  • Tell your doctor if you get blurred vision or eye pain. These could be signs of serious eye problems, which could lead to permanent vision loss if not treated.
  • Tell your doctor if you have had breathing problems or lung issues (like inflammation or fluid in the lungs) after taking hydrochlorothiazide. If you develop severe shortness of breath after taking this medicine, seek immediate medical attention.
  • Tell your doctor if you have had swelling of the face, lips, throat, or tongue (an allergic reaction).
  • This medicine may increase your risk of certain skin and lip cancers (non-melanoma skin cancer) with long-term, high-dose use. Protect your skin from sun and UV rays while taking this medicine.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Effectiveness is reduced in renal impairment. Consider risk of orthostatic hypotension and hyponatraemia and -kalemia. (updated 19.5.2025)
