How to Use

Dosage

The usual starting dose is 1.25 mg or 2.5 mg once daily. Your doctor will adjust your dose based on your condition.

Method

Take the tablets whole with water, at the same time each day. Do not crush or chew them. You can take them with or without food.

Important

The maximum daily dose is 10 mg. For some conditions, your doctor may recommend taking it in two doses per day.

Possible Side Effects

COMMON
  • Headache or feeling tired
  • Dizziness (more likely when starting or increasing dose)
  • Fainting, abnormally low blood pressure (especially when standing up quickly)
  • Dry, irritating cough, sinus or bronchitis inflammation, shortness of breath
  • Stomach or gut pain, diarrhea, indigestion, feeling or being sick
  • Rash (possibly with swelling)
  • Chest pain
  • Muscle cramps or pain
  • Higher than normal potassium levels in blood tests
RARE
  • Balance problems (vertigo)
  • Itching and unusual skin sensations (numbness, tingling, burning)
  • Change or loss of taste
  • Sleep problems
  • Feeling down, anxious, restless, or more nervous
  • Blocked nose, breathing difficulties, or worsening of asthma
  • Swelling in your gut (intestinal angioedema) causing stomach pain, vomiting, and diarrhea
  • Heartburn, constipation, or dry mouth
  • More urine production than usual
  • More sweating than usual
  • Loss or decrease of appetite
  • Faster or irregular heartbeats
  • Swollen arms and legs (sign of fluid retention)
  • Flushing
  • Blurred vision
  • Joint pain
  • Fever
  • Inability to have sex (men), decreased sex drive (men and women)
  • Increased number of certain white blood cells (eosinophilia) seen in blood tests
  • Changes in liver, pancreas, or kidney function shown in blood tests
  • Feeling unsure or confused
  • Red and swollen tongue
  • Severe peeling or flaking skin, itchy lumpy rash
  • Nail problems (e.g., loose or detached nails)
  • Skin rash or bruising
  • Spots on the skin and cold limbs
  • Red, itchy, swollen, or watery eyes
  • Hearing problems or ringing in the ears
  • Feeling weak
  • Lower red blood cells, white blood cells, platelets, or hemoglobin in blood tests
  • Increased sensitivity to sunlight
  • Difficulty concentrating
  • Swelling in the mouth
  • Too few blood cells in blood tests
  • Lower than normal sodium levels in blood tests
  • Dark, concentrated urine, feeling sick or unwell, muscle cramps, confusion, and fits (may be due to inappropriate ADH secretion)
  • Color changes in fingers and toes when cold, with tingling or pain when warm (Raynaud's phenomenon)
  • Breast enlargement in men
  • Slowed or impaired reactions
  • Burning sensation
  • Changes in smell
  • Hair loss

Important Warnings

  • Do not take Cardace if you are allergic to ramipril, any other ACE inhibitor, or any of its ingredients. Signs of a severe allergic reaction (angioedema) include swelling of your face, lips, tongue, or throat, which can make it hard to swallow or breathe.
  • Do not take Cardace if you are pregnant, planning to become pregnant, or breastfeeding. It is not recommended for children under 18 years of age.
  • Tell your doctor if you have heart, liver, or kidney problems, or if you are on dialysis or have very low blood pressure.
  • Inform your doctor if you are taking other medications that might interact with Cardace, especially sacubitril/valsartan, aliskiren, or other blood pressure medications.
  • Cardace can cause dizziness, particularly when you start treatment or increase your dose. Do not drive or operate machinery if you feel dizzy.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
