How to Use

Dosage

Your doctor will decide your dose, usually starting with 1.25 mg or 2.5 mg once a day.

Method

Take the tablet by mouth with liquid, whole, with or without food, at the same time each day. Do not crush or chew.

Important

The maximum daily dose is 10 mg.

Possible Side Effects

COMMON
  • Headache or feeling tired
  • Dizziness (more likely when starting or increasing dose)
  • Feeling faint, low blood pressure (especially when standing up quickly)
  • Dry, irritating cough, sinusitis or bronchitis, difficulty breathing
  • Stomach or gut pain, diarrhea, indigestion, feeling or being sick
  • Skin rash (with or without swelling)
  • Chest pain, muscle cramps or muscle pain
  • Higher than normal potassium levels in blood tests
RARE
  • Balance problems, spinning sensation (vertigo), itching and unusual skin sensations (numbness, tingling, burning)
  • Loss or change of taste, sleep problems, feeling down, worried, restless, or more nervous
  • Swelling in the gut (intestinal angioedema) with symptoms like abdominal pain, vomiting, diarrhea
  • Heartburn, constipation, dry mouth, more urine than usual, more sweating than usual, loss of appetite
  • Faster or irregular heartbeats, swollen arms or legs (signs of fluid retention), flushing, blurred vision
  • Joint pain, fever, impotence in men, reduced sex drive in men and women
  • Changes in liver, pancreas, or kidney function shown in blood tests, higher number of certain white blood cells (eosinophilia)
  • Feeling unsteady or confused, red and swollen tongue, severe peeling or flaking of skin, itchy, lumpy rash, nail problems, rash or bruising, spots on skin and cold limbs
  • Red, itchy, swollen, or watery eyes, hearing problems or ringing in the ears, feeling weak
  • Lower numbers of red or white blood cells, platelets, or hemoglobin shown in blood tests, more sensitive to sunlight
  • Difficulty concentrating, swelling in mouth, too few blood cells or lower sodium levels in blood tests
  • Dark urine, feeling unwell or sick, muscle cramps, confusion and seizures (due to too much ADH hormone), changes in finger and toe color when cold (Raynaud's phenomenon)
  • Breast enlargement in men, slowed reactions, burning sensation, changes in smell, hair loss

Important Warnings

  • Do not take Cardace if you are allergic to ramipril, other similar medicines (ACE inhibitors), or any other ingredients. Signs of a severe allergic reaction (angioedema) include swelling of your face, lips, throat, or tongue, which may cause difficulty swallowing or breathing.
  • Do not take Cardace if you are pregnant (especially after the first 3 months) or breastfeeding, as it can harm your baby.
  • Do not take Cardace if you have severe kidney problems, are on dialysis, or have very low or unstable blood pressure.
  • Tell your doctor if you have heart, liver, or other kidney problems, or if you have recently lost a lot of body fluids (e.g., from vomiting or heavy sweating).
  • Tell your doctor if you are having surgery, dental work, or treatment for allergies (desensitization), as your Cardace dose might need adjustment or temporary stoppage.

  • Avoid taking Cardace if you have diabetes and kidney problems and are taking aliskiren.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Reduce the dose in moderately severe renal impairment. Monitor renal function and potassium concentrations during the pharmacotherapy. Avoid concurrent use of NSAIDs. (updated 14.4.2025)
