How to Use

Dosage

For high blood pressure and chest pain, your doctor will usually start you on 5 mg once a day and adjust the dose to find what works best for you. For heart failure, your doctor will start with a very low dose, like 1.25 mg once a day, and slowly increase it over time. If you have severe kidney or liver disease, your doctor will set your dose.

Method

Take Bisoprolol Accord tablets whole with water in the morning, with or without food. Do not chew or crush the tablets. If needed, tablets can be split in half along the score line.

Important

The highest daily dose for high blood pressure and chest pain is usually 20 mg. For heart failure, severe kidney disease, or severe liver disease, the highest daily dose is 10 mg.

Possible Side Effects

COMMON
  • Feeling tired, weak, dizzy, or having a headache (these are often mild and go away in 1-2 weeks, especially when treatment starts or the dose changes for high blood pressure/chest pain).
  • Feeling cold or numb in your hands or feet.
  • Low blood pressure, especially in people with heart failure.
  • Stomach or bowel problems like feeling sick (nausea), throwing up (vomiting), diarrhea, or constipation.
  • A slow or irregular heartbeat (more common in people with chronic heart failure).
  • Worsening of heart failure symptoms (also more common in people with chronic heart failure).
RARE
  • Sleep problems or feeling down (depression).
  • Feeling dizzy when standing up quickly.
  • Breathing difficulties, especially if you have asthma or long-term lung disease.
  • Muscle weakness or cramps.
  • Hearing problems, allergic reactions like a stuffy or runny nose, itching, redness, or rash. Severe allergic reactions (swelling of face, throat, or trouble breathing) require immediate medical help.
  • Reduced tear production (can be a problem for contact lens wearers).
  • Liver problems that may cause yellow skin or eyes, or unusual results in liver blood tests.
  • Problems with erections.
  • Nightmares, seeing or hearing things that aren't there (hallucinations), or fainting.
  • Eye irritation and redness (like pink eye), hair loss, or new/worsening psoriasis (a skin condition).
  • Worsening of blood circulation problems in your legs.

Important Warnings

  • Do not take if you are allergic to bisoprolol or any other ingredient in this medicine.
  • Do not take if you have very severe asthma or other serious, long-term lung diseases.
  • Do not take if you have serious heart problems like sudden worsening of heart failure, a very slow heart rate, or very low blood pressure.
  • Do not take if you have severe circulation problems in your arms or legs, which can make your fingers and toes feel numb, cold, or turn pale or blue (like Raynaud's syndrome).
  • Tell your doctor if you have diabetes, kidney or liver problems, mild asthma, or a thyroid problem.

  • This medicine might harm your baby if taken during pregnancy. It is not recommended to breastfeed while using this medicine.
  • Be careful when driving or using machines, especially when you first start taking this medicine, if your dose changes, or if you drink alcohol. It might affect your ability to concentrate.

Use in Elderly Patients

Category A

Suitable for older adults. Reduce the dose in severe renal impairment. Significant adverse effects include bradycardia and orthostatic hypotension. Consider interactions , especially with drugs affecting the conduction system of the heart. (updated 20.1.2025)
