How to Use

Dosage

Your doctor will decide your dose. For adults and adolescents over 12 years old, the usual daily dose is 1–5 million IU (0.6–3 g), divided into 4–6 doses.

Method

A doctor or nurse will give you this medicine as an injection into a muscle or directly into a vein (intravenously) as an infusion.

Important

For severe infections, the daily dose can be up to 40 million IU (24 g).

Possible Side Effects

COMMON
  • Changes in laboratory test results.
RARE
  • Allergic reactions, hives, severe skin reactions with fever and blisters (e.g., erythema multiforme, exfoliative dermatitis), fever, joint pain, mouth sores, black hairy tongue, nausea, vomiting.
  • Nervous system disorders, including seizures (especially with high doses or fast infusion).
  • Electrolyte imbalances caused by fast high-dose infusions.
  • Diarrhea (can be severe and a sign of gut inflammation).
  • Kidney disease, abnormal levels of protein or blood in urine, sediment in urine, decreased urine output or inability to urinate.
  • Increased certain white blood cells (eosinophils), decreased white blood cells, hemolytic anemia (reduced red blood cells), bleeding disorders, low platelet count.
  • Severe local reactions when injected into a muscle, especially in babies.
  • Sudden widespread rash with pus-filled blisters (AGEP), flat red rash, measles-like rash, redness of skin (erythema), swelling under the skin (angioedema, often in face or tongue), blistering skin condition (pemphigoid).
  • Serum sickness-like reaction (allergic reaction with fever, swollen lymph nodes, local redness, itching).
  • Jarisch-Herxheimer reaction (sudden fever, chills, rash, headache, muscle and joint pain, tiredness/fatigue) - particularly in Lyme or syphilis treatment.
  • Brain disorders with seizures and loss of consciousness (metabolic encephalopathy), liver inflammation, reduced bile flow.

Important Warnings

  • Do not use if you are allergic to penicillin or other similar antibiotics.
  • Stop taking this medicine and seek urgent medical help if you get a serious allergic reaction, such as a rash, itching, trouble breathing, swelling of your face or throat, fever, or joint pain.
  • Tell your doctor if you have kidney or liver problems, as your dose may need to be adjusted.
  • Inform your doctor if you have a heart condition or severe electrolyte imbalances, such as problems with sodium, calcium, potassium, or chloride levels.
  • Tell your doctor if you have epilepsy, fluid in the brain, or brain inflammation, as there's a higher risk of seizures during treatment.

  • Contact your doctor immediately if you get severe or persistent diarrhea during or after treatment, as it could be a sign of a serious gut inflammation.
  • If the medicine is given too quickly through infusion, it can cause brain seizures.
