How to Use

Dosage

For adults and adolescents 12 years and older, the usual daily dose is 0.6–3 grams, divided into 4–6 doses. Doses for children and infants are determined by a doctor based on weight.

Method

This medicine is usually given by a doctor as an injection into a muscle or a vein. When given into a vein, it can be a quick injection or a short infusion (usually 30–60 minutes).

Important

For severe infections, the dose for adults can be increased up to 24 grams per day.

Possible Side Effects

COMMON
  • Changes in laboratory test results
  • Allergic reactions (e.g., mild rash)
  • Hives
  • Fever
  • Joint pain
  • Inflammation of the lining of the mouth
  • Tongue inflammation or black hairy tongue
  • Nausea and vomiting
RARE
  • Electrolyte imbalances
  • Nervous system disorders, including seizures
  • Diarrhea or inflammation of the large intestine
  • Kidney disease, abnormal amounts of protein or blood in urine, or decreased urine output
  • Severe local reactions (especially in infants if the medicine is injected into a muscle)
  • Changes in blood cell counts (e.g., increased eosinophils, decreased white blood cells, anemia due to red blood cell breakdown)
  • Blood clotting disorders or prolonged bleeding time
  • Decreased number of platelets (thrombocytopenia)
  • Maculopapular or measles-like rash
  • Hypersensitivity reaction called serum sickness (symptoms include fever, swollen lymph nodes, redness, and itching at the injection site)
  • Jarisch-Herxheimer reaction (sudden fever, chills, skin redness, headache, muscle and joint pain, fatigue)
  • Brain dysfunction (metabolic encephalopathy), with seizures and loss of consciousness
  • Liver inflammation or decreased bile flow
  • Blistering skin disease (pemphigoid)

Important Warnings

  • Contact your doctor immediately if you develop a widespread rash, blisters, skin peeling (especially around the mouth, nose, eyes, and genitals), or flu-like symptoms such as fever, rash, and swollen lymph nodes.
  • Stop using this medicine and seek urgent medical help if you experience a severe allergic reaction, such as a rash, itching, difficulty breathing, tightness in the chest, or swelling of the face, lips, tongue, or eyelids.
  • Tell your doctor if you are allergic to penicillin or other antibiotics, or if you have had a severe allergic reaction before.
  • Inform your doctor if you have kidney or liver problems, epilepsy, heart disease, or diabetes.
  • Warning: Too rapid intravenous injection can cause seizures.
