How to Use

Dosage

Typically, one 2.5 mg or 5 mg tablet is taken twice a day, depending on your condition.

Method

Swallow the tablet with water, with or without food, at the same time each day. If you have trouble swallowing, you can crush it and mix with water, apple juice, or applesauce.

Important

The maximum daily dose varies by condition and doctor's instructions, but can be up to 20 mg in certain initial treatment cases.

Possible Side Effects

COMMON
  • Bleeding, such as bruises, nosebleeds, blood in urine, or bleeding from gums, stomach, or intestines.
  • Feeling tired or pale due to anemia.
  • Nausea (feeling sick).
  • Low blood pressure, which can make you feel faint or cause your heart to beat fast.
  • Skin rash or itching.
  • Changes in blood tests, for example, related to liver function or blood platelets.
RARE
  • Severe allergic reactions, including swelling of the face, lips, mouth, tongue, and/or throat, or difficulty breathing. Seek immediate medical help.
  • Bleeding into muscles, eyes, brain, spinal cord, or lungs. Bleeding after surgery or from the vagina, or blood in sputum (coughing up blood).
  • A decrease in the number of blood platelets (cells that help blood clot).
  • Abnormal liver function tests or jaundice (yellowing of skin or eyes).
  • Hair loss.

Important Warnings

  • Do not take this medicine if you are allergic to apixaban or other ingredients.
  • Do not take this medicine if you have heavy bleeding.
  • Do not take this medicine if you have a condition that increases your risk of severe bleeding (e.g., stomach ulcer, brain bleed).
  • Do not take this medicine if you have liver disease that increases your bleeding risk.
  • Do not take this medicine if you are on other blood thinners, unless your doctor has instructed you to switch treatments.

  • Tell your doctor if you have an increased risk of bleeding (e.g., platelet problems, very high blood pressure, over 75 years old, or weigh less than 60 kg).
  • Tell your doctor if you have severe kidney disease or are on dialysis.
  • Tell your doctor if you have or have had liver problems.
  • Tell your doctor if you have had a spinal procedure or injection.
  • Tell your doctor if you have an artificial heart valve.
  • Tell your doctor if you have a rare condition called antiphospholipid syndrome.
  • Do not stop taking this medicine without talking to your doctor, as your risk of blood clots may increase.
  • If you experience swelling of your face, lips, mouth, tongue, and/or throat or difficulty breathing (allergic reactions), contact a doctor immediately.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Consider renal impairment, age and body weight when determining the dosage. Consider potential interactions especially with other drugs that increase the risk of bleeding. (updated 19.9.2025)
