How to Use

Dosage

The typical dose is one 5 mg tablet twice a day, or one 2.5 mg tablet twice a day if your doctor decides a lower dose is needed.

Method

Swallow the tablet with water, with or without food. Try to take your medicine at the same time each day. If you cannot swallow the tablet whole, it can be crushed and mixed with water, apple juice, or applesauce.

Important

The usual maximum daily dose is 10 mg (two 5 mg tablets), unless your doctor prescribes a different regimen for specific conditions, such as the initial treatment of blood clots.

Possible Side Effects

COMMON
  • Bleeding, for example in the eyes, stomach or intestines, rectum, urine, nose, gums, or as bruising and swelling.
  • Anemia, which can cause tiredness or paleness.
  • Low blood pressure, which can cause feeling dizzy or a faster heartbeat.
  • Nausea.
  • Blood tests may show increased gamma-glutamyl transferase (GGT) levels.
RARE
  • Bleeding in the brain or spinal cord, mouth (or blood in sputum), abdomen or vagina, post-surgery bleeding (including bruising, swelling, wound discharge), from hemorrhoids, or blood in stool/urine detected by tests.
  • Abnormal liver function or increased levels of certain liver enzymes.
  • Increased bilirubin, which can cause yellowing of the skin or eyes.
  • Reduced platelet count (can affect blood clotting).
  • Hair loss, itching, rash.
  • Allergic reactions (hypersensitivity) which can cause swelling of the face, lips, mouth, tongue, and/or throat, and difficulty breathing. Contact a doctor immediately if these symptoms occur.
  • Bleeding in the lungs or throat, behind the abdomen, or into a muscle.
  • Rash that can form blisters and look like small target lesions (erythema multiforme).
  • Inflammation of blood vessels (vasculitis), which can lead to rash, raised, flat, red, round spots under the skin, or bruising.
  • Bleeding in the kidneys, sometimes with blood in the urine, leading to kidney problems (anticoagulant-related nephropathy).

Important Warnings

  • Do not take if you are allergic to apixaban or any other ingredients.
  • Do not take if you have severe bleeding.
  • Do not take if you have a condition that increases your risk of severe bleeding, such as a stomach ulcer or recent brain bleed.
  • Do not take if you have a liver disease that increases your bleeding risk.
  • Do not take if you are already on other blood-thinning medications, unless your doctor tells you to switch or for specific procedures.

  • Do not take if you are pregnant or breastfeeding; consult your doctor.
  • This medicine is not recommended for children and adolescents weighing less than 35 kg.
  • Always inform your doctor if you need surgery or a medical procedure.
  • Tell your doctor if you have antiphospholipid syndrome (an immune system disorder).

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Consider renal impairment, age and body weight when determining the dosage. Consider potential interactions especially with other drugs that increase the risk of bleeding. (updated 19.9.2025)
