How to Use

Dosage

Typically, one 2.5 mg or 5 mg tablet, taken twice a day. Your doctor will tell you the exact dose.

Method

Swallow the tablet whole with water, with or without food, at the same time each day. If you have trouble swallowing, you can crush it and mix with water, a glucose solution, apple juice, or applesauce just before taking.

Important

The maximum daily dose is usually 10 mg, but your doctor will tell you your exact dose.

Possible Side Effects

COMMON
  • Bleeding (such as easy bruising, nosebleeds, bleeding gums, blood in urine or stool, bleeding from wounds or after surgery, or bleeding in the eyes, stomach, intestines, rectum, or vagina).
  • Anemia (feeling tired or pale due to low red blood cells).
  • Nausea (feeling sick to your stomach).
  • Low blood pressure (may cause dizziness or a fast heartbeat).
  • Skin rash or itching.
  • Hair loss.
  • Changes in blood tests (such as liver function tests or a low number of platelets, which help blood clot).
RARE
  • Severe allergic reactions (swelling of your face, lips, mouth, tongue, or throat, or trouble breathing – get help right away).
  • Bleeding in the brain or spinal cord.
  • Blistering or target-shaped skin rash (called erythema multiforme).
  • Inflammation of blood vessels (vasculitis), which can cause a skin rash or bruises.
  • Bleeding in the kidneys that can lead to kidney problems.

Important Warnings

  • Do not take this medicine if you are allergic to apixaban or any of its other ingredients.
  • Do not take this medicine if you have severe bleeding.
  • Do not take this medicine if you have a medical condition that increases your risk of serious bleeding, such as a stomach ulcer or a recent brain bleed.
  • Do not take this medicine if you have liver disease that increases your risk of bleeding.
  • Do not take this medicine if you are already taking other blood-thinning medications (e.g., warfarin, rivaroxaban, dabigatran, or heparin), unless your doctor specifically tells you to switch.

  • Tell your doctor if you have a higher risk of bleeding due to conditions like a bleeding disorder, uncontrolled high blood pressure, being over 75 years old, or weighing less than 60 kg.
  • Tell your doctor if you have severe kidney disease or are on dialysis.
  • Tell your doctor if you have an artificial heart valve.
  • Tell your doctor if you have a condition called antiphospholipid syndrome, which increases the risk of blood clots.
