How to Use

Dosage

Your doctor will determine your exact dose, but it is typically 75-150 mg daily for depression and OCD, starting at 10 mg and increasing slowly to 100 mg for panic disorder, and 10-150 mg daily for chronic pain.

Method

Swallow the tablets whole or divided with plenty of water; do not chew or crush them. You can take this medicine with or without food, but avoid grapefruit and cranberry juice.

Important

The maximum daily dose is generally 150 mg, but your doctor will advise you on your specific limit.

Possible Side Effects

COMMON
  • Feeling sleepy, tired, dizzy, or restless
  • Dry mouth
  • Increased hunger, gaining weight
  • Blurred vision, trouble focusing your eyes
  • Headache
  • Shaking (tremor)
  • Problems with sexual function
  • Feeling sick (nausea)
  • Constipation
  • Trouble urinating or holding urine
  • Sweating a lot
  • Feeling confused, not knowing time or place
  • Seeing or hearing things that aren't real (hallucinations)
  • Feeling worried, restless, or having trouble sleeping (like insomnia, nightmares, or yawning often)
  • Mild mania or feeling aggressive
  • Trouble remembering or focusing
  • Hot flashes
  • Large pupils (the black center of your eye)
  • Low blood pressure (especially when standing up quickly)
  • Irregular heartbeat
  • Vomiting
  • Diarrhea
  • Stomach problems
  • Decreased hunger
  • Changes in liver blood tests
  • Allergic skin reactions, itching, skin sensitive to sunlight
  • Breast swelling or milk production (in men or women)
  • Changes in taste
  • Ringing in the ears (tinnitus)
RARE
  • Worsening of mental illness symptoms (psychosis)
  • Seizures or fits
  • Poor coordination (ataxia)
  • Heart rhythm problems
  • High blood pressure
  • Changes in brain waves (EEG changes)
  • Very high fever
  • High pressure in the eyes (glaucoma)
  • Inability to empty your bladder
  • Problems with the heart's electrical signals
  • Liver inflammation (hepatitis) or yellow skin/eyes (jaundice)
  • Swelling in parts of the body or all over
  • Hair loss
  • Problems with a hormone that controls body water
  • Allergic lung inflammation
  • Severe allergic reaction (anaphylaxis)
  • Changes in blood cell counts (like more white blood cells) or red-purple spots on skin (purpura)
  • Feeling an intense need to move (akathisia)
  • Repeated, uncontrolled movements
  • Muscle damage
  • Higher levels of a hormone called prolactin in the blood
  • Serotonin syndrome (a serious reaction with symptoms like agitation, confusion, diarrhea, high fever, high blood pressure, heavy sweating, and fast heart rate)
  • Delayed or absent ejaculation in men
  • Increased risk of bone fractures

Important Warnings

  • Do not use this medicine if you are allergic to clomipramine or similar medicines, use certain antidepressants (MAOIs), or have recently had a heart attack or severe heart disease.
  • Tell your doctor immediately if you have thoughts of harming yourself or suicide, or if your depression gets worse.
  • Tell your doctor if you have ever had seizures or fits.
  • Tell your doctor if you have an irregular heartbeat or any other heart problems.
  • Tell your doctor if you have schizophrenia or any other mental health disorder.

  • Tell your doctor if you have liver or kidney disease.

  • Tell your doctor if you are taking buprenorphine or other opioids for addiction treatment.
  • This medicine is generally not recommended for children and adolescents under 18 years old.
  • Do not drive or operate machinery if this medicine makes you drowsy, blurs your vision, or affects your attention.
  • This medicine can make your skin more sensitive to the sun. Avoid direct sunlight and wear protective clothing and sunglasses.
