How to Use

Dosage

Your dose of Actilyse depends on your body weight and the medical condition being treated.

Method

Actilyse is administered by a doctor or nurse as an injection into a vein, typically starting with a fast injection followed by a slower infusion over a period of time.

Important

The total maximum dose for a single treatment is typically 90 mg or 100 mg, depending on your condition.

Possible Side Effects

COMMON
  • Heart failure (treatment may need to be stopped)
  • Bleeding in the brain (when treated for stroke, treatment may need to be stopped)
  • Fluid in the lungs
  • Bleeding from damaged blood vessels, such as bruising
  • Low blood pressure
  • Chest pain (angina pectoris)
  • New heart attack
  • Bleeding in the brain (when treated for heart attack, treatment may need to be stopped)
  • Cardiac arrest (treatment may need to be stopped)
  • Shock due to heart problems (very low blood pressure, treatment may need to be stopped)
  • Bleeding in the throat
  • Bleeding in the digestive tract, such as bloody vomit, bloody stools, or gum bleeding
  • Bruising caused by bleeding in tissues
  • Bleeding in the urinary tract or genitals, which may cause blood in the urine
  • Bleeding or bruising at the injection site
RARE
  • Lung-related bleeding, such as coughing up blood or airway bleeding (treatment may need to be stopped)
  • Nosebleed
  • Irregular heartbeat after blood flow to the heart is restored
  • Damage to heart valves or the wall separating heart chambers (treatment may need to be stopped)
  • Sudden formation of blood clots in the lungs (pulmonary embolism), brain (cerebral embolism), or other parts of the body (systemic embolism)
  • Ear bleeding
  • Bleeding into the sac surrounding the heart (treatment may need to be stopped)
  • Internal bleeding behind the abdominal cavity (treatment may need to be stopped)
  • Formation of blood clots that travel to other parts of the body (symptoms depend on the affected organ, treatment may need to be stopped)
  • Allergic reactions such as hives, rash, breathing difficulties due to narrowed airways, swelling under the skin or mucous membranes (angioedema), low blood pressure, or shock (treatment may need to be stopped)
  • Bleeding in the eyes
  • Nausea
  • Severe allergic reaction (e.g., life-threatening anaphylaxis, treatment may need to be stopped)
  • Nervous system symptoms such as seizures, speech problems, confusion or severe confusion (delirium), anxiety, restlessness, depression, or changed thinking (psychosis)
  • Bleeding in internal organs, such as the liver (treatment may need to be stopped)
  • Formation and travel of cholesterol crystals in the body (symptoms depend on the affected organ, treatment may need to be stopped)
  • Bleeding requiring blood transfusion
  • Vomiting
  • Fever

Important Warnings

  • You are allergic to alteplase or any other ingredient in this medicine.
  • You have a high risk of bleeding, such as a bleeding disorder, serious bleeding in any part of your body, bleeding in the brain, or very high uncontrolled blood pressure.
  • You are taking blood-thinning medicines, unless specific tests show they are not significantly affecting your blood clotting.
  • You have had brain or spinal surgery, major surgery, or a severe injury within the last 3 months.
  • You have recently had a large blood vessel punctured, external heart massage, or given birth within the last 10 days.

  • You have a history of bleeding in the brain or a stroke of unknown cause, or a recent stroke caused by a blood clot (within 6 months), except for the current stroke being treated.
  • You are under 16 years old.
